Family Birth Center preadmission registration

You should receive a welcome packet folder from your doctor with a registration form and pre-paid postage envelope. Even if you’ve delivered at the birth center before, please complete it and mail it to us, or drop it off with the Patient Access registrar or at the registration desk at Community Hospital.

Preadmission interview

Schedule your preadmission interview online six to eight weeks before your baby is due. The preadmission interview is a phone call that lasts about 30 minutes. The interview allows us to answer any questions you have about your hospital stay.
